A relic of both power and mystery, the Memento Mortem Wand allows its wielder to peer into the past and witness a soul’s final moments.
Memento Mortem Wand - 5e
Wand, legendary (requires attunement by a spellcaster)
This wand has 7 charges. The wand regains 1d6 + 1 expended charges daily at dawn up to a total of 7 charges. If you expend the wand’s last charge, roll a d20. On a 1, the wand crumbles into ashes and is destroyed.
Moment of Death (1 charge). While holding the wand, you can use your action to expend one charge to target a dead body within 5 feet of you. A frozen illusion appears, depicting the exact moment and location of that target’s death and recreating all of the scene’s details, suspended in time in a 30-foot radius centered around the target. This illusion lasts for 10 minutes. If the body was moved postmortem, the illusion appears where the target died.
Spells. While holding the wand, you can use your action to expend some of its charges to cast one of the following spells (save DC 18): haste (2 charges), slow (2 charges), circle of death (4 charges), time stop (6 charges).

This wand allows you to send a creature tumbling through time or view the area around a dead body at the time of its death.
Activate Cast a Spell; Frequency once per day, plus overcharge; Effect You cast cast into time, except you can target only one creature in the spell’s area. Alternatively, you can target one dead creature in the area. If you do, instead of the spell’s normal effects, an illusory, motionless recreation of the area around the creature when it died appears at the location of the creature’s death. (If the body was moved postmortem, the illusion appears in the location where the creature died.) The scene includes all objects and creatures that were in a 30-foot emanation of the creature when it died. The scene is obviously illusory and lasts for 10 minutes.
