Not every cultist in your game should feel like a throwaway minion.

If you’re running cosmic horror, eldritch corruption, or an invasion from beyond the planes, you need enemies that feel like they’ve been changed by the thing they serve. You want something more dangerous than a basic robed fanatic, but not so overwhelming that it overshadows the rest of the encounter.

That is exactly where the Void-Touched Thrall shines.

This is a strong, flexible cult enemy that works beautifully in low- and mid-level adventures. It can serve as a frontline enforcer, a terrifying sign of growing corruption, or the first real hint that the cult your players are dealing with is connected to something far worse.

What Is a Void-Touched Thrall?

Cultists are always looking for power, and the ch’thuun have plenty of it to offer.

When these vile entities begin infesting a world, they don’t just feed on the living. They also make use of those willing to serve them. The most devoted cultists may receive a “blessing” from the ch’thuun regis: a glimpse into the void between planes. But this gift is not free. That brush with the void twists the cultist’s body, warps the mind, and binds their loyalty completely to the regis.

The result is the Void-Touched Thrall.

These creatures are no longer ordinary humanoids. They are transformed servants, physically altered to resemble their masters and granted fragments of ch’thuun power. Some develop insectoid eyes. Others grow chitinous plates, clawed limbs, or spider-like movement. Some even awaken to telepathic whispers that bind them more tightly to the will of the void.

That combination makes them ideal as recurring cult soldiers, lieutenants, temple guards, or terrifying proof that the cult’s promises of power are horrifyingly real.

Magic Resistance. The void-touched thrall has advantage on saving throws against spells and other magical effects.

Ch'thuun Mutation. Void-touched thralls are physically warped in some way to resemble their masters. Each cultist will have one of the following mutations:

Type I. These thralls have insectoid eyes and possess the Compound Eyes trait.

§  Compound Eyes. The void-touched thrall has advantage on Wisdom (Perception) checks that rely on sight.

Type II. These cultists have one of their arms replaced by a ch'thuun arm and claw. They can replace one of their scimitar attacks with a claw attack.

Type III. These thralls have their legs replaced with the legs of a ch'thuun. They have a climb speed of 30 feet and possess the Spider Climb trait.

§  Spider Climb. The void-touched thrall can climb difficult surfaces, including upside down on ceilings, without needing to make an ability check.

Type IV. These thralls have their minds awakened by the egg's mutating power. They have telepathy to a range of 120 ft.

Type V. These thralls have chitinous plates over a portion of their bodies. They have a natural armor class of 15 when not wearing armor.

Actions

Multiattack. The void-touched thrall makes two scimitar attacks.

Scimitar. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 6 (1d6 + 3) slashing damage.

Claw (Type II Only). Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 12 (2d8 + 3) slashing damage. If the target is Medium or smaller, it is grappled (escape DC 11).

Light Crossbow. Ranged Weapon Attack: +5 to hit, range 80/320 ft., one target. Hit: 7 (1d8 + 3) piercing damage.

How to Use the Void-Touched Thrall

The biggest strength of the Void-Touched Thrall is not raw damage. It is variety.

Because each thrall can have a different mutation, you can use several of them in the same encounter and make the fight feel much more dynamic than a standard mob of cultists. One might cling to the ceiling and fire down with a crossbow. Another might rush the front line with its claw, trying to grapple a weaker target. Another might serve as a sentry, using its strange insectoid eyes to spot intruders before they ever reach the cult’s lair.

That makes these creatures excellent for cult strongholds, corrupted temples, underground nests, or any adventure where the players are slowly uncovering the spread of something alien and malignant.

They also work well as transitional enemies. At CR 2, they are strong enough to feel dangerous at lower levels, but not so overwhelming that they have to be saved for late-game encounters. More importantly, they hint at a larger threat. When your players see that ordinary cultists are being physically remade by the power they serve, it immediately raises questions about what kind of beings the cult is actually worshipping.

That is where the real value of this monster lies.

Where This Monster Really Shines

The Void-Touched Thrall is especially effective in adventures featuring:

  • cosmic horror and eldritch corruption
  • cults serving alien or extraplanar entities
  • creeping body horror and mutation
  • dungeon crawls where enemy variety matters
  • low- and mid-level encounters with a disturbing twist

They are also excellent as evidence of escalation. At first, the players may only hear rumors of missing people or strange cult activity. Then they encounter a Void-Touched Thrall and realize the corruption is no longer theoretical. It is here, it is spreading, and it is changing people.

That can do a lot of storytelling work for a relatively simple stat block.

Final Thoughts

The Void-Touched Thrall is a great example of how a lower-CR creature can still feel memorable when it carries the right flavor and design. It is not just a stat block you toss into an encounter to fill space. It is a sign of corruption, a tool for building tension, and a way to show your players that something far worse is waiting behind the curtain.

Used well, these creatures can make a cult feel truly dangerous. Not because the cult is large, but because it is being changed by something otherworldly and evil.

And that is often much more unsettling than a simple sword fight with a few robed fanatics.
